Floor tiles raised from part of kitchen floor

Home Insurance looking for full report on why this has happened to determine if repair costs will be covered by insurance. 12 tiles lifted off floor, grout and adhesive had fallen away. Removed to make safe that area. Approximately effected area is 6ft x 4ft Kitchen floor is concrete and tiles fitted directly onto this. No sign of leak in kitchen, no condensation on windows, no cracks on walls. Property bought from Builder 5 years ago and tiles were laid before moving in. Looking for source of problem, repair and refit original tiles.
