Lay concrete slab as foundation for log cabin

Prepare the ground and lay a concrete slab with these dimensions: Slab: rectangular; 4.2m (l) x 3.2m (w) x 100mm (d) Waterproof membrane sub base: 50mm compacted hardcore Slab to protrude 50mm above ground level and screed to be level (this will be the foundation for a 4 x 3 m log cabin). Contractor to provide all materials and dispose of any excavated material. Work could start as early as Nov 21st 2011 but must be completed by December 9 2011. Site is a gravelled area in a garden, adjacent to main access road with a wide footpath that would have room for a skip. However, a hedge and stone wall are between the site and road which would prevent a direct pour from a concrete lorry. Access would be arranged by removing 1 or 2 6x6ft fence panels from an adjacent wooden fence to give a ~10m run by barrow between the road and site.
