New warehouse slab over existing slab

New ~700 Sq.M industrial warehouse floor slab over existing slab. Will need hardcore base over existing slab (approx 50mm to 300mm hardcore base due to existing slope), dpm, 50mm insulation, min 150mm fibre or reinforced concrete slab. Power float finish. There are around 4 drain/manhole covers that will need to built to new floor finish and three ramps to doorways because of the new higher floor level. Please price all inclusive i.e. materials, plant, labour and rubbish disposal (in any) etc. (Alternatively, contact me if you wish to quote for plant and labour only)
