Taking up and replacing a slipped concrete path

Hi The path outside our house (at the top of stairs up from pavement and at the top of steep garden) has come away from the house and has slipped. It needs digging up and relaying with probably some sort of support added between the new path and garden to stop the slippage happening again. The area is approx 1m wide by 5 m long. The property is between Honor Oak Park and Forest Hill. I can send pictures of the problem if required. thanks
